Mental Health Collective LLC is a group of independent mental health providers based at 2961 Yarmouth Greenway Dr., Suite 2, Fitchburg, Wisconsin 53711, in the greater Madison area. The collective describes its model as bringing together independent clinicians under shared infrastructure so that providers can focus on client care rather than administrative tasks. Clients contact individual providers directly, as the collective does not manage referrals centrally.

Eleven independent practices operate under the collective. Licensed clinicians include Barb Perkins LCSW (Affirmed Family Psychotherapy), Amanda Case LCSW (Compassionate Healing Psychotherapy), Shaina Gordon LCSW (DBT Counseling), Lori Pulkrabek LCSW (Inner Strength Counseling, virtual only), Kaleb Nelson LCSW (Fermata Psychotherapy), Amanda Meyer LMFT (Meyer Therapy), Joe Leonard LMFT (Palm Tree Family Therapy), Laura Long LMFT-T (Rainbow Mental Health), Golshan Motamedi LMFT (Rhythmic Connections), Nichole Fritts LPC-IT (The Empowered Place), and Gabi Jump LPC (The Simplicity in Authenticity Counseling).

Services across the collective include individual, couples, family, and group therapy. Presenting concerns addressed include anxiety, depression, trauma, ADHD, disordered eating, PTSD, life transitions, and relationship difficulties. Amanda Meyer brings more than ten years of experience in couples therapy with a focus on infidelity and intimacy, while Lori Pulkrabek holds more than twenty years of specialization in disordered eating. Several providers identify as LGBTQ+ affirming, and kink- and polyamory-aware practitioners are listed within the collective.

Therapeutic modalities available across providers include Dialectical Behavior Therapy (DBT), Radically Open DBT (RO-DBT), Gottman Method, Prepare/Enrich, Acceptance and Commitment Therapy (ACT), CBT, attachment-based therapy, and solution-focused therapy. Many providers offer telehealth in addition to in-person sessions.

Insurance acceptance varies by provider. Plans listed across the collective include Dean, Quartz, Forward Health, Aetna, BCBS/Anthem, ComPsych EAP, Partners in Behavioral Health, and Alliance/UW SHIP. Sliding scale fees are available through select providers.
